What To Bring With You:

Insurance Cards
Picture ID
All Medications you are on - please bring the bottles, or an accurate list that is current
A family member may accompany you in order to assist with your medical history

What to Anticipate:

a urine specimen will be requested for analysis and microscopic examination
ultrasound of your kidneys may be performed in the office if necessary

Additional Information:

Parking is available, but sometimes tight; if you come in a large vehicle or truck, please be considerate and seek street-side parking.
Please arrive a little early to complete paperwork if not done ahead of your appointment; about 10 minutes is sufficient.
If you are unable to keep your  appointment, please call ahead to reschedule; your missed appointment will prevent another patient from being seen.
If you do not show for your scheduled appointment, a rescheduling fee will need to be reconciled at the front desk.
As the waiting area has limited seating, only one family member per patient can comfortably be accommodated at most visits.
Cell phones should be silenced in the office as a consideration to other patients and staff.
